Disk Warning "1 umformat" with 6 beeps preceding it

My NVR quit recording and when I restart the system I hear 6 beeps, the system loads and then displays an error warning. "Disk Warning" - "1 Unformat"
I re-formatted the disk through the NVR but nothing changes and nothing is getting recorded.
Any suggestions on what to do next ?
