Flying Guide Dog on Delta

On July 2 I bought a ticket to fly from Anchorage to Amsterdam with my Guide dog whom I have made this trip at least 6 times and never really had any issues. Fill out the proper paper work and permits and it has been smooth sailing. This year something has changed. I filled out the 2 DOT forms (dot and attestation) and now Delta wants you to upload them to "my Trips" which seemed simple enough. The Dot form went straight through. The attestation form however did not as there is not away to upload on to their system. I keep getting emails from a special services address saying a "form is missing". No other explanation. I tried scanning form and replied to that email address only to have the exact reply back " missing form". Called the accessibility desk on 4 different occasions and had them submit over the phone the same form. They got the same response. I gave up and canceled my ticket to see my new grand daughter and will try to travel in the spring.  This is an international flight exceeding 8 hours. One more thing,which the web site didn't mention is Delta says I need a CDC import form to come back to Alaska. I called the CDC and they were confused also as this is a round trip. My travel agent called a sales agent at Delta and she told them that she thought I need the 2 forms for out bound and inbound which means 4 different forms. There is nowhere on Delta's site that says this. Also  the rejection emails I keep getting are always the same which I suspect is an A I generated response from a 3rd part vendor. The Delta agents suspect the same....very frustrating. Any suggestions where and who I can escalate this up the Delta ladder?  Erin (best Guide dog ever and the knucklehead she lets me feed her) bryan

How about bypass Delta and see if there's another airline that will get you there with less hassle.  Also, post in social media; sometimes that does wonders.  Also, what about complaining to DOT or DOJ.  Oh, I almost forgot:  the current administration doesn't care about civil rights.--Matt & fancy.

I was going to say the same.  As a travel agent, I would advise you to find a better airline.  My preferred two are American and United.

How Frustrating.  Hopefully, Delta will look into the issue for their own business reasons. 

 

Below is information I copied from the CDC’s website on their new form required for any dog entering the U.S. by air.  Hope this helps.

 

Form Required by the Federal Center for Disease Control and Prevention (CDC) when Returning to the U.S. Internationally

  •  For all dogs returning to the U.S., including service dogs, you'll need to complete the CDC Dog Import Form and provide a receipt. 
  • CDC Dog Import Form
  • Rabies Vaccination: Ensure your dog meets the CDC's rabies vaccination requirements, especially if traveling from a high-risk country. 
  • Microchip: A microchip is typically required for service dogs entering the U.S. 
  • Health: Your dog must appear healthy upon arrival. 
  • Approved Ports: Ensure you're arriving at a CDC-approved port of entry.

Hi Bryan,

That sounds quite frustrating. Perhaps, try filling out Delta's Accessibility Request Form. That is how I was asked to register Osaka, my guide dog, and I could have sworn there was the ability to upload multiple documents. (I had filled it out for a domestic flight so did not upload it). Your required CDC form should be the Dog Import Form, and your form for entry into the Netherlands should be the EU Health Certificate. Check the U.S. Department of Agriculture APHIS site for the requirements for the Netherlands, and the CDC Dog Import Form page for the Dog Import receipt online form.
