Forms not loading in certain workflows

It seems some forms are not properly loading in certain workflows and just spin forever. There is nothing wrong with my workflow itself as it has worked fine in the past. This appears to be a bug of some sort on the Ultradox server side not wanting to read or load something with no console error output. Any ideas?

Make sure you are not out of credits with UD.  

Also, try reloading the UD triggers on those Google Forms.
