Disconnect from Google Forms to start fresh with loading data

Besides removing the Google Form input option from the Ultradox file, is there anything else I need to do in order to restart integrating Google Forms as inputs for a file?

I am having troubles getting data from a form over to Ultradox and it's so hit or miss I wonder if it's just years of issues all connected.

Mainly I want to ensure that form data is being collected and my debugging is showing it's not. I've triple checked variable names, but no luck.

Would prefer to just start fresh.
