Can anyone explain how QR code based FIDO Authentication work?

The QR code is only used to link the authenticator to the client. It is not QR-based authentication.
FIDO Cross-Device Authentication uses the newly defined "hybrid" transport which is defined in the upcoming CTAP 2.2 specification. It uses a combination of BLE (for proof of proximity and metadata exchange) and a network-based tunnel provided by the authenticator.
