Failure in network code registration

Dear all,
I'm trying to register a new permanent network code (C7) on the request form. However I got the error message "There were errors in the submitted information. Please fix them and try again." and I wasn't not able to understand where the error is.
Could anybody kindly give me an advice or indicating a contact?

I apologize if this is not the appropriate mailing-list, however I was not able to find on the FDSN web site an e-mail contact.
