2023 BIOMETRIC SCREENINGS

At Larimer County, we care about our employees. That’s why we have partnered with Catapult Health to provide free, quick and confidential preventive health checkups right here at our workplace. Appointments are now available throughout February and these checkups are available to all employees and spouses covered under the medical plan.
If you think you’re healthy, fantastic! This 30-45 minute checkup will help confirm that. However, if you have any warning signs or risk factors, this checkup can help you identify what’s going on. A licensed Catapult Health Nurse Practitioner will even help you create a personal action plan for improving your health. Employees can use up to an hour of clinic leave for this appointment.
The Biometric Screening is REQUIRED to qualify for the 2024 Well Rate, a $480 annual premium reduction ($40/month), in addition to the other required wellness activities (i.e. online Wellbeing survey, Tobacco Free Affidavit and/or Tobacco Free program, and reporting 2 Preventive Care Exams completed between 10/1/2022 – 9/30/2023).
If your spouse is covered under the Larimer County medical plan and they complete the biometric screening AND meet 3 out of 5 biometric criteria, you can earn an additional $480 annual premium reduction ($40/month) for 2024. If both the employee AND the spouse qualify for the annual premium reduction, it could mean a reduced premium cost of $80/month. A separate appointment will need to be made for your spouse.

WELLNESS RATE BIOMETRIC CRITERIA
(Must meet 3 out of 5 criteria*)
- Waist circumference of ≤ 35 inches for women and ≤ 40 inches for men OR complete an In-Body Scan at the Wellness Clinic
- Blood Pressure below 130/80 mmHg
- Total Cholesterol below 200 mg/dL
- Triglycerides below 150 mg/dL
- Blood Glucose below 100 mg/d OR A1C below 7.0
* If 3/5 of the biometric results are not met, you will still receive credit for meeting the criteria as long as you complete a virtual visit with a Nurse Practitioner during your Catapult screening.

If you prefer to have your screening completed with your personal physician, please have them complete the physician form found here. You must then upload the form directly on your Navigate portal to complete this requirement.
For all current employees (hired by 7/15/2023), the deadline for completing all required activities is September 30, 2023. You can access the full 2023 Wellness Program Guide here.

Please note, this is the last year that Larimer County will be offering onsite biometric screenings. Starting in 2024, the program requirement will change so that you will have to complete your biometric screening either at the Wellness Clinic or through your primary care provider.
We will provide more information about this as we get closer to 2024.

Please be assured that the only information Larimer County will receive is whether or not a participant meets the overall incentive requirement, not how a person meets it or which criteria they’ve met. Larimer County will not receive any identified medical information. Hence, Larimer County will remain HIPAA compliant.
