Minimum Systolic's that are zero or abnormally low

Hello!  I am wondering what to do with systolic BP's that are showing as zero or any other's that are very low such as 7, 12, 22, etc...

Would we leave those as is?

Would we chart review each and manually enter the next subsequent minimum (not sure if we have time to do that.  There are many).

Has anyone else come across this? If so, what are your thoughts?
