Add OK or FAIL after other browsers where you have tested this issue:
Opera: 25.0.1619.84037

Android: 4.0.4

What steps will reproduce the problem?
1. Create a Web Page, and add an input field on a form like this:
<input type="file" accept="image/*;capture=camera" name="some name">
2. Submit the form

What is the expected result?
Upload will be send to server

What happens instead?
Chrome Crashes.

Additional information (IMPORTANT)
Ive added an DEMO application URL to reproduce the problem.
This problem only occours with newer versions of Chrome, (and Opera) on Old
Androids (4.0.x). On Android 4.4 it works as expected.

Obs: In the version 30.x of Chrome, this feature works fine on Android 4.0.x

I have the problem with de Chrome version 38.0.2125.102 and 38.0.2125.114.
