Cas 6.6.8, login flow executes twice

Doesn't always happen but occasionally I see my custom login webflow get executed twice.it usually has no impact but it's causing SQL errors with dup primary key violation and log shows step in the flow executing within the same second which is impossible for a user to do.

Anyone facing this problem?
