Objectives: The aim was to assess the effectiveness of administration of
Anti-D immunoglobulin at 28 weeks and 34 weeks or more of pregnancy on the
incidence of immunisation when given to Rhesus negative mothers without
Anti-D antibodies.
Search strategy: Trials were identified from the Cochrane Pregnancy and
Childbirth Groups Specialised Register of Controlled Trials.
Selection criteria: All published, unpublished and ongoing randomised
trials which met the following criteria: Participants: Rhesus negative
women without Anti-D antibodies at 28 weeks gestation. Intervention: Anti-D
immunoglobulin given at 28 weeks or more of pregnancy. Outcome measures:
Development of a positive Kleihauer test, subsequent development of
immunisation, and neonatal morbidity.
Data collection and analysis: DATA EXTRACTION
Data was extracted from each trial by the author without blinding.
Main results: A positive Kleihauer was found less commonly at 32-35 weeks
and after delivery of a Rhesus positive infant in women treated with Anti-D
at 28 and 34 weeks. There was a clear reduction in the incidence of
immunisation at 2-12 months following delivery in primiparae and
primigravidae who had received Anti-D antenatally.
Conclusions: Administration of Anti-D immunoglobulin at 28 weeks' and 34
weeks' gestation can significantly reduce the risk of immunisation during
or immediately after a first pregnancy. The cost effectiveness of such
treatment needs further study.
