I may need to get a small cavity removed, and I was wondering if
anyone knew of any dentists in Tokyo who use air abrasion/laser
methods for this. (Anything to avoid the drill!) Doesn't matter if
they speak English or not -- I just need an excellent and TRUSTWORTHY
dentist. I've heard so many horror stories, but I refuse to believe
that there are no good ones in Tokyo!

In the long run I always found it cheaper to fly back to the states and have
any dentistry done. "Japanese dentistry". Now that's funny.
If it's small enough to be sand blasted, I would try brushing with a
fluoride
gel after your regular brushing for about a week to see if it will heal
itself.
I know it sounds strange, but my brother just avoided a sand blasting
by using fluoride.
Ask someone at your place of work who has kids about their family dentist -
children are pretty good as barometers of friendly or not-so-friendly
dentists. My - go to when something goes wrong with my teeth in Japan -
dentist is a woman (gentle hands - especially with the hypodermic) and
always has plenty of kids waiting to see her.
For a regular filling I think you'll be ok - just make sure that the surgeon
reaches for the anesthetic before he/she reaches for the drill. A lot of
dentists have this - you're a big man and this is just a small filling so no
need for an anesthetic - attitude. Screw that! If they refuse you just get
up and leave. Expect to be called back in the following day to have the
filling polished - now....that can make your stomach turn.

My dentist did some pretty major work in my mouth a couple of years
back - the sounds of the drill were terrible but he never hurt me. And
he speaks a little english. Just my wife's families local dentist but
he's a good guy. Doesn't help you (I'm in Nagoya) but just proof that
not all Japanese dentists are bad.
Next week he's promised to teach me to brush my teeth :-/
