I have a Sandisk SD 128Mb card
It was fine until yesterday when I moved pictures from it to my PC via a
card reader. Now it says that the card is not initialized. I can read it via
the card reader, and have formatted there, but no luck.
Any suggestions?

You will have to format it in the camera for it to work there. This always
happens to me if I erase the pictures from the card reader.
