What steps will reproduce the problem?
1. Disable the camera in Settings.app -> Restrictions
2. Run an app that includes the ZXingWidgetController.
3. Crash

What is the expected output? What do you see instead?
I would expect an error message. Instead the app crashes.

What version of the product are you using? On what operating system?
SVN Rev 2806. iOS 6.1.3

Please provide any additional information below.
In ZXingWidgetController - (void)initCapture the captureInput value isn't
being checked for nil.

Fixed in r2868 though I didn't put in the alert view. I think that should
be up to the app. The caller can look at the captureSession to see if the
widget was able to go through the AVFF setup dance.
