  When I submit form, I see &lt;br&gt; &lt;p&gt;Fatal error: Class &#39;PDO&#39; not found in /home/sirsoftc/public_html/ &lt;br&gt; _mpiasta/form/objects/class.da tabase.php on line 21 &lt;br&gt; &lt;p&gt;And right before that, it said: &lt;br&gt; &lt;p&gt;Could not find wff_misc.php. For some reason, the form file looked &lt;br&gt; like : &lt;br&gt; &lt;p&gt;include_once(&amp;quot;../configuration .php&amp;quot;);

  Not sure if this will solve your issue but I noticed two problems with &lt;br&gt; your code. I believe you should be using &amp;quot;s and not &#39;s -- maybe &#39;s &lt;br&gt; work but I&#39;ve always used &amp;quot;s. Also there should be a space before &lt;br&gt; the / at the end of the tag: name=&amp;quot;submit&amp;quot; /&amp;gt; not name=&amp;quot;submit&amp;quot;/&amp;gt; &lt;br&gt; &lt;p&gt; - k.

  I agree. I am having a difficult time getting the setup to work. &lt;br&gt; And the video tutorial is for a local host. I would recommend that &lt;br&gt; you show how it is done to a live remote site. &lt;br&gt; Thanks for the service you are providing.
