Unsubscribe function enhancement

Wishlist: I'd love to offer my subscribers some standard checkboxes
for reasons why they're unsubscribing which they can opt to use (but
not mandatory).
Eg:
We're sorry to see you leave this subscription. Is it because:
- you've changed email address
- the emails aren't of interest to you
- you're reducing inbox clutter
- you object to the content
- other (text field for leaving subscriber to fill in)
It would save me questioning whether it's my content unnecessarily
every time! What do you think?
