I have taken this as far as possible...

I am on OSX 10.6.

I have a dead m key so I am trying to make z/Z into m/M.

Any idea where I have gone wrong?

I'm not sure what the problem is. It looks from the screen shot that you
created (twice) a new keyboard layout based on the US keyboard layout, but
we can't see what you have done with it since then. It should be
straightforward to assign m and M to the z key, though be careful to do it
for caps lock as well as shift.

That said, if you have a non-working key on your keyboard, the better
option is to get it fixed (unlikely to be feasible in most cases) or
replaced, or simply use another keyboard, as USB keyboards are pretty cheap.
