(found accidentally, no relation etc, but got me wondering) to
effectively lengthen the stem. I can see that one disadvantage with this
system is that of having another bit to tighten which can thus become
loose.
Anyone out there using stem extenders or would everyone go for a
straight replacement?

A straight replacement, every time - if the picture is any indication, a
setup like that is an accident waiting to happen (IMO)[1]. Look at it another
way, one of those extender doofers will cost you just under 16 quid - you
could buy a new (possibly adjustable) stem for that, or less.

[1] - Call me a cynic, but the `extenders' look very much like the extenders
you get with Spinaci-u-like aerobars.

When 3T "The" stems are about 15 quid and go as long as 135mm, why
mess around with a stem extender?
