Today's Electrical Torygraph (Wednesday 14th) has a slideshow of aerial photos of London by Jason Hawkes.

The Fleet flows into the Thames just south of err... Fleet Street. Under
Blackfriars Bridge having been channelled under New Bridge Street.

It's therefore possible that Blackfriars (Circle/District) also spans
the river.

The current view on Google Maps shows the station under construction,
exposing the alignment of the track.

That's the key point -- how many stations have full (not just emergency)
entrances and exits on both sides of a surface (ie, not tunnelled) river?

You mean separate entrances either side? That's true of St Pancras/Fleet

And what about canals and trams? The "Station Street" terminus for the
Nottingham tram spans the canal.

I believe at Sheffield you can.  AIUI the Sheaf runs below the tracks between platform 1 and 2.

The River Irk runs in culvert somewhere below Manchester Victoria.  
Victoria and Exchange, including the former Europe's longest railway
platform, used to span the River Irwell just above its confluence with
the Irk.  I don't know which station the bridge was in, maybe both.

One of the Athenian metro stations, think it might be the deep level of
Monastiraki, has a classical-era culvert that was discovered in the building
of the station, in which quite a lot of water still flows.  You can walk
directly off the platform into the excavation and see the open water that
flows under the tracks.

However the sound of flowing water on the platforms can be uncomfortable for
some, as there are no toilets on the metro system.
