Being a brash American, I don't expect to have the elevator all to
myself, nor do they - so no one is offended when another person
enters. However, we do say "excuse/pardon me" when passing someone on
an escalator or on one of those flat people movers at the airport. :)

neither do we (I've never known anybody apologise for entering a lift
I was in *unless* it was pretty much full and everybody had to squeeze
up). We do tend to say "sorry" at lot though.

cannot recollect anybody doing that here except to tourists standing
on the walking only side (left) on London tube (subway) escalators.

Unless I just haven't got to it yet, there's no "unable to leave a
lift or cashpoint without saying 'thank you'"

They're about the only people I /don't/ say sorry to. It's more
"exCUSE me!". Oh, and people blocking the exit to the platform by
trying to push on when there are people getting off the tube at
Ox.Circ (particularly).

I've noticed that pedestrians on NYC sidewalks do it too.
