I actually just encountered a finished project that does exactly what you 
described. Tumble Tree <http://tumble-tree.tumblr.com/> does it very well. 
You may want to look into how they do it for ideas. The tumblr API call for 
getting info on a single post would be a good place to look. See here<http://www.tumblr.com/docs/en/api/v2#posts>. 
Particularly, the "notes_info" parameter will be of interest to you.
