Citing a map

I'd like to cite a Polish map for a research paper. I can't read
a word of Polish, so I don't know where to begin.

The map was published in 1958. My best guess is that

"Polska Rzeczpospolita Ludowa: Mapa administracyjna"

is the title, and

"Panstwowe Przedsiebiorstwo Wydawnictw Kartograficznych"

is the publisher. Other text written near the legend includes:

"Warsawa - styczen 1958"

"Redaktor H. Cytowski"

"Druk PPWK - W-wa, Solec 18, Zam. 5160/3/B/58 - 6.Ill.57 - CW -
31519 - 25200 - 80x90 mapowy 100 g"

If someone could advise me on how to write the citation for my
paper, I would appreciate it. Thank you.
