Attached is a screen shot showing failure to read back a file that had been just saved?
Any ideas as to why this might happen.

This is saying that the requested file was not found by the SoxService. This is unexpected given that the file you're requesting is the app itself.
If you have access to the device's debug output, do you see any failure messages there?
Had you just written the app to the device, or made any other recent changes to the app in the device?
Can you still connect to the app from the workbench? If so, how much memory is available in the device?
Can you capture the sox traffic that was sent just prior to the failure?
