Questionnaire design howlers

Two of us here are doing up a guidesheet on preparing questionnaires
and data collection sheets. We have some wonderful examples
of poorly designed items. Does anyone have good examples of
items that either caused problems or were fixed before they
could cause problems?
We're also trying to draw up a useful set of guidelines, like
- collect one item of information at a time
- do not spread an item of information over several questions
- do not use more response categories than a person can distinguish
and so on. Anyone got favourite maxims?

interest.
