Where do military code names come from?

Just curious...where do military code names come from?  Or how are they
generated?

Some are obviously political (e.g., "Iraqi Freedom").  Or obviously
named (e.g., "Desert Storm", "Rolling Thunder").

But I'm thinking of...say, the code names for the Normandy beaches.
"Sword," "Juno," "Omaha," etc.  Or other operation names - e.g.,
"Market Garden", or a nuclear test like "Buster-Jangle", or Vietnam's
Linebacker, Ranch Hand, etc.

Is there some protocol for thinking these up?  Is there some guy who
pulls names out of a hat?

Someone has to think of the name...is this something staff assistants
do, or...?
