It is a very short, soft, high-frequency "snapping" sound.
It is audible especially during high output (as after sex or workouts);
otherwise it's relatively rare;
It is only audible around the 2nd-3rd left intercostal spaces (no text
mentions that a MVOS is audible anywhere but at the apex);
I have only heard it with her in the supine position;
It is definitely distinct from normal physiological splitting of S2. I
know this because a) I have heard both occuring simultaneosly, which
produced a "triplet effect," and b) because it generally tends to be
louder during expiration. Her split S2, on the other hand, is always
present and is quite "textbook".
She also has an occasional innocent flow murmur around the same
location.
She is in excellent physical condition, a regular runner with a resting
HR in the low 50's and BP around 110/70;
Her physicals are always normal; no internist seems to have picked this
stuff up (so much for their training and prowess). But it's definitely
there.
