What is likely to have caused the thermal fuse to blow - failure of the
mechanical cut-out?
There are three cut-out outs. One is the thermostat, the second is the
mechanical one just above the heater elements and the third is the
thermal fuse also above the heater elements.

Some folks with little understanding of how heating appliances (and many
other devices) work, will place objects on heaters to warm those items (wet
towel, jacket, hat, etc).
Any objects placed on many styles of heating appliances will substantially
decrease the needed free air flow.
One could test each temperature limiting device, but it's often much easier
to just replace the thermal fuse with a proper replacement device*, and make
sure that the interior is clean, and that the appliance has adequate free
air flow at the location where it's to be used.
*Use of an inappropriate safety device is hazardous.
