Be aware that map2sqlite does not create an mbtile sqlite db.   The files are stored in osm slippy tile format.   For me an excellent overview of the differences see: http://alastaira.wordpress.com/2011/07/06/converting-tms-tile-coordinates-to-googlebingosm-tile-coordinates/

btw, I just put up code to convert osm slippy tile files to mbtiles format.
