When updating from 1.2.5 to 1.3.4 the "
paster upgrade-db production.ini
" command fails with the provided error message.
Is there anything I can do to make this go smoother?

Exactly as error stated, there are duplicate entries for users_group_id, permission_id make sure that pair are unique, and then try again.

When I opened the SQLite DB in a stand alone viewer, there were indeed duplicate permissions. Not sure how that happened though...
