Here is a list of all-out Test innings in which the openers were the
only two batsmen to achieve double-figure scores.

Eng (105) v Aus Melbourne(4) 1907-08 JB Hobbs 57/G Gunn 13
Aus (84) v Eng Manchester 1956 CC McDonald 32/JW Burke 22
NZ (67) v Eng Leeds 1958 LSM Miller 26/JW D’Arcy 11
SAf (96) v Aus Cape Town 2011-12 JA Rudolph 18/GC Smith 37

Note: All matches, except the last on the list, were lost by the
batsmen’s sides.

I imagine that innings in which numbers 9 and 11 are the only batsmen to
reach double figures are even rarer. :)

You are right! The Cape Town Test is the only such instance.

http://www.espncricinfo.com/ci/engine/match/62447.html saw a near
miss: McKibbin (no 11) scored 16, and Trumble (no 9) was second top
scorer with 7*.
