Assuming that there have been no other 12th men involved, I believe that

would have been Adam & Ben Hollioake vs. Australia at Trent Bridge in 1997.
They were the fifth siblings to appear together for England:
EM, GF & WG Grace 1 Test 1882
CT & GB Studd 4 " 1882/83
F & GG Hearne 1 " 1891/92
DW & PE Richardson 1 " 1957
Note that a third Hearne brother played in the 1891/92 match - for South
Africa.
