Undoubtedly more jobs are being created thruout the world. Conan O'Brian brought forth the FACT that no time in history has there been 2 popes. :-)

Seriously, I have yet to see any change in my investments, healthcare, or my friends losing jobs. As a matter of fact, the few that did lose their jobs are now working, sooooo, unless the sequester makes a some kind of drastic change, things are looking upward in the future. YMMV

My mileage varies. I dropped by my doctor's office last Friday to
check on a rumor I'd heard. It was true. I've got to find a new
primary care doctor. A sign on her door said that she was closing her
practice on June 1 due to "changes in the medical program". She's
going to become a hospitalist.

As near as I can tell, that means she'll earn a salary and not have be
concerned about having to have a staff trying to make sure her
practice is complying with all the stuff in the Obamacare train wreck.
And Obamacare aside, she can doctor without having to devote time to
office administration.

Sounds like a win-win. If she is a good doctor, then she may become a great doctor due to the fact she'll get in more "practice time". I quit practicing after I became perfect. Altho my wife may disagree with that.

The internist group founded by my internist went that route about a year
ago. They have offices in the medical pavilion attached to their employer's
hospital. They only admit patients to their employer's hospital. I expect
to see a lot more of this as independent physicians get tired of paying a
huge overhead to make sure they comply with all the rules and get paid and
hospitals move to lock up patients via their physicians.

Unlikely. There will still be the pressure to take and see more patients
from the insurance companies. If the physicians decline to take more new
patients, the insurance company will drop them from their plan, resulting
in too few patients. The insurance companies still make the rules.

many groups affiliated this way around here. It puts them in a
combined billing and record keeping system, multi location laboratory
services, different insurance scheme, and connected to a large
teaching hospital and medical school.

It's not so easy for the insurance companies to jerk around somebody

You need to look up hospitalist. Hospitalists take care of patients in
hospitals full time. They don't have "offices" in which to see and
acquire new patients. They don't send people to hospitals - they
doctor them after they're sent there by some other doctor - or brought
there after being pried out of their wrecked Toyota.

There are part-time hospitalists, I suppose, because my doctor is
currently working as a hospitalist in a neighboring county while she
still has her office here. That will change June 1 when she closes her
office and becomes a full-time hospitalist.

Once that happens, I can no longer call her and ask, "Hey. When can
you see me to find out what this thing is that's growing out of my elbow?"

patients saying that they, the practice, were joining XYZ hospital would be
moving to the hospital building, and would no longer be admitting patients

So, what's your doctor going to do do when she closes her current
practice, can't you just make appointments at her new job. You sure are
making this complicated. Just call her new group practice, and make and
appointment.

Or get in one hell of a car wreck and you'll get to see her without an

I think there's an epidemic of incomprehension running through this

She's going to be a hospitalist. You don't make appointments with
hospitalists. Hospitalists don't do appointments - either directly or
indirectly. They tend to patients in hospital beds in hospitals. I've
made this clear several times.

That might be - if I end up in the right bed in the right hospital at
the right time.

I went to her office today to see about getting a copy of my medical
records. Her husband - the office manager - is who I talked to. He
confirmed that about 80-90% of her (their) decision to become a
hospitalist was the crap they'd have to keep up with and comply with
as a result of Obamacare if she continued as a primary care manager.

He also confirmed - without my even having to ask - that she would not
be available after Jun 1, and that if I didn't find another doctor by
then, my options would be pretty much limited (in this town) to urgent
care and the emergency room. I told him, "Yeah, that figures. Just
when I finally get her trained, she leaves".

I've put my wife in charge in finding another doctor - she's the one
with connections here.

And it's going to get worse, for all of us. Medicare patients are
directly in the cross hairs. I know a couple of guys in here think that
because it hasn't happened yet, it won't. Dreamers.

We changed to Medicare Advantage in 2011. In that time we've had 5
PCP's. the first one was really good, but she moved. The second one
worked in an office full of nit wits, so we moved. The next office
had/has a good staff, but the first 2 PCP's moved to another practice,
so we're on number 5. The problem is that I've been told Medicare
Advantage is going away shortly, so we'll most likely be back in the
hunt in 2014.
