Text of Perfectionist is pretty clear that the gaining of
the blood is not optional. However, after the first action,
you don't know that the second action will succeed with no
reactions being played. So, you can't choose to delay the
Perfectionist blood gain, can you? Or can you?

AFAICS, Freak Drive is played in the same "after resolving the action"
window as the blood gain from Perfectionist, so surely you can order
these effects as you choose?

I'm pretty sure that there's no option to delay Perfectionist under
any circumstances. The first time in a turn the conditions are met,
it happens. (As stated and confirmed by LSJ, though, there's no
reason you can't play the freak, then put the Perfectionist blood on.)

So if vamp has Rutor's hand and Perfectionist, takes 2
actions, the second of which costs a blood, xe can't use the
Perfectionist on the 2nd action? I.e., the blood flow MUST
look like this:
Vamp full
Action 1 (no reaction cards played)
Perfectionist can be used in this window, or not at all
again this turn.
Vamp blood: capacity +1 blood, 1 blood drains off
OR
Vamp blood: no change (Perfectionist not used)
Action 2 costs 1 blood (no reaction cards played)
Vamp ending blood: Capacity -1 blood.
That's correct, right?

You don't "use" the Perfectionist. It (mandatorily) triggers on its
side without asking for your permission.
Action 1
If successful, no reaction cards played: Gain 1 from Perfectionist.
Blood excess drains off, as usual.
Action 2
If successful, no reaction cards played, and if Perfectionist hasn't
been triggered previously: Gain 1 from Perfectionist. Blood excess
drains off.
In your example, the vampire would end with capacity - 1 blood.
