In order to add items to the pouch, 'w'ield them. You can wield wands
(when they fire about ten times as often per turn as normal, creating a
lethal machine-gun effect), potions (can save an equipment slot if you
only have one of a potion - but note that pouch items *don't stack*)
and scrolls. I don't think you can wield anything else, but you will be
presented with a menu of all the potential items in your possession
when you issue the command in any case.
In order to use pouch items, press the appropriate key ('r', 'q', 'a'
etc.) and use the '/' key to toggle between inventory and equipment.
Pouches count as equipment, and if you have any relevant items equipped
- depending on the command you just issued - you will be able to select
them at the prompt.

Next to the RNG, the keyboard buffer probably kills more Angband
characters than people realize.
