The first time was in a 3-player game, which means the odds of making
such a hand were greatly increased anyway. I wrote about it at
http://www.sloperama.com/mahjongg/column188j.htm
The second time was in a Nintendo handheld game, which means the odds
could have been greatly askew.
The third time, though, was at the Open European Mahjong Championship
in Copenhagen. I wrote about it at http://www.sloperama.com/mahjongg/column325.htm
The guy I made the hand from was later found to be cheating.
I played mahjong since 1975 (+/-) (I mean seriously involved in
regular playing).
The only form of play is HKOS.
I have had personally won these hands:
Many = more than 10
The Hidden Treasure [?] OR All Hidden Pungs 全暗刻 [6-10]
All Honours [Many]
All Terminals [Many]
All Green (Imperial Jade) [3 to 5]
Three Great Scholars (Large Three Honours) [Many]
Four Large Blessings (Four Large Lucks) [?] Big Four Winds 大四喜 [6-10]
All Kongs (Four by Four Peace) [5+ but I don't recall this to be a
predefined special hand in HKOS]
Nine United Sons (Calling Nine Cards) / Nine Gates 九莲宝灯 [5-6]
Thirteen Unique Wonders (The Thirteen Odds) [Many - Also see "record
historic moment" next]
Heavenly Peace (Hand from Heaven) [10+/-]
Earthly Peace [3-5]
Moon from the Bottom of the Sea [3-5]
Plum Blossom on the Roof [Many]
Scratching a Carrying Pole [Many]
Kong on Kong [Many]

As to my "record historic moment," the following one has been
imprinted on my brain forever:
We play a game that we must draw to the last available pai on the
wall, and the player who draws the last pai but could not declare win
must also expose one pai. [The term "expose" can mean "discarding a
pai to the floor" or "making a kong" within his hand, the latter is
where a pai must also be disclosed to other players.]
I was holding a hand of pais calling for the "Thirteen Unios 十三幺",
with only Red Dragon that's missing. It had now come to the last pai
on the wall and it was the turn of my opposa (the player at the
opposite side of my seat) to draw that last pai. After having drawn
the last pai, he (I still remember "Pang" is his name!) had thought
for over a minute and with a smile at me, he disclosed FOUR RED
DRAGONS all at the same time with an attempt to make an "unfolded kong
暗杠" - No doubt these four Red Dragons were designated for me, but I
need just one!
Now I won the Thirteen Unios with many more extra fans: From Robbed
Kong, won as if I had selfmade. It was my record moment and had
happened over 20 years now. And the full record of the play can be
viewed online through this link (a step by step online record of the
full game):

PS: Of course the "link of step by step online record of the full
game" here is just a joke - But it also is *my serious pioneer idea of
feature* the ideal future online mahjong games (programs) should
cover. Technically it is possible, right? Could any computer and
programming genius, or game designer or developer, comment on this?
