Does anyone recognize this design? Or even the designer or manufacturer?

My mother stitched it for a cousin. Cousin's friend would like to make
one. Mom no longer has the chart and so can't tell cousin what to look
for on e-bay or any place else. We think it may have been a kit, and
Mom remembers the chart was in four sections.
