I might have totally missed a period when things might
have been 'less frugal', but seems to me that "good
enough" has always applied to the spending priorities
given to the USMC.

dialysis machine for several hours

I doubt that.

It's almost unheard of in the developing world as the expertise to
maintain the machines doesn't exist outside a hospital.

Did he attend the local hospital for dialysis every week?

I somehow doubt that as well.

There's a big teaching hospital in Abbottabad, but it mainly teaches
gynaecologists.

As the people there spell dialysis 'dialosis' on their web site and the
page has no content I don't think they're terribly good at it.
