Contract clause about reporting problems

Hello all,

How can one write a contract clause that mandates that the client has
to communicate any and all problems to the service provider in order
that the issue/problem be fixed? In other words, how can one ensure
that if a client sits on a problem for 20 days without reporting it, s/
he cannot say the system was *down* for 20 days while the reality is
that the service provider was never warned. It should be a quick 2-3
liners for anyone who "speaks" legalese.

I think the way ours handles this is just to state that our
responsibility begins once it is reported. That solves the problem
99% of the time
