This one, with some modification. go for either <Heh, ZWJ, Dot, Space, Sheen, Dot>, or <Heh, ZWNJ, Dot, Sheen, Dot>, depending if you are tight with space or not.

There is no Dot at the end.
Also forق‍ـ م
I don't remember seeing Dot at the end in any of these cases in (old) books.

But I don't see any reference for that part of your reference! Is there a historical reason or specific standard?

One should bear in mind that style guides provide recommendation. The information provided in such documents are based on best practices the author believes to be valid, relevant, and mostly widespread. You can consult other available style guides (see the references part of that document) and decide what is best for your usage.
