Name Change practices around userID

I am interested in hearing what your practice or policy is regarding name changes of employees in your organization.  If you assign userIDs that incorporate the person name, do you change the userID when a person’s name changes, or do you simply change the name and make them keep the same userID?

 

For example, Cindy Jones has a userID of “cjones”, but changes her name to Cindy Smith.  Would you change her name to Cindy Smith AND change her userID to “csmith”, OR would you change her name to Cindy Smith but leave her userID as “cjones”?

 

We used to require userIDs to stay the same but adopted the practice of changing them several years ago.  Now we are finding instances where changing the userID is problematic and wish to revert to our original practice of keeping them the same.  I would be interested in hearing what others do.
