OLD INFECTED ELBOW FRACTURE

Dear All

42 yr old male

Road accident april 2010- treated by ORIF internal fixation elsewhere- infection occurred.

 went to another hospital- the implants removed debrided and then after wound was clean Re –ORIF  and bone grafting.

Pt was on follow up  at that hospital till they told him it needs another surgery after which elbow may become stiff

Pt is clerk by profession and can do his activities of daily living without much of  problems

Clinical examination:  No tenderness – almost full ROM

15 deg varus- valgus instability

X-rays- show a gap in distal one third humerus and on flexion and extension of the elbow it s the distal fragment which moves.

Elbow seems ankylosed from what can be seen on x-rays

Pt is quite asymptomatic

Should anything be done? If I get a union with whatever technique  pt is going to be really unhappy as his elbow may become quite stiff.

I have told him for the time being no active treatment- if the implant fails later and is symptomatic then fix the elbow which in effect will arthrodesing the joint

Please leave it alone. If some symptoms occur due to the implant, think of just removing it and early ROM. Gaurav N Bhalla
