Dear all

77  yr old lady-

H/O fall at home 8 months ago . subtrochantric # femur . Treated elsewhere with DCS when medically fit.

Since last 3 months pt cannot walk – severe pain

In addition OA knees also

X-ray jan2010 shows – head screw cur out , collapse of the # into varus [ I don’t know if another # neck of femur has occurred]

Now what is to be done? I have suggested a THR .

Requesting opinions

THR is the best choice, maybe you'll have problem with the femoral canal, go slowly.

The head looks good enough, so i'd remove hardware, provide valgus reduction and fix the femur with gamma nail or similar implant.
