Orthopaedic Residency Logbook

I am looking for an Orthopaedic Residents' Logbook as we are in the process of updating and improving our residency program.
I would appreciate it if anyone with access to such a book could provid me with a copy, in English of course.

and contacting Prof Angus Wallace (Angus....@rcsed.ac.uk) regarding this. He has setup the surgical logbook system in the UK and it has been very well designed.
