Dear Friends

46 yr old lady- well to do pt- – pain [R] ankle. no significant medical problems

H/O fall July 2011- bimaleolar fracture -treated in in another place by an orthopod with POP cast  for 8 weeks[ for unexplained reasons] – removed it went to another orthopod who continue with cast treatment for another 4 weeks.

The foot appears laterally translated and ? an abnormal medio-lateral mobility at supra maleolar area. The Ankle Range of Motion  is DF=20° and PF =20°-painless !!!. Pt is walking doing her work with pain and discomfort.

X-rays show lateral shifted ankle mortise with displace medial and lateral malleolus.

I am planning to fix the medial malleolus with a Tension Band wiring and lateral with plate.

I am putting up this case to ask the opinion of the House with regard to any difference in management or fixation
