I just saw a post on the Listserv about an upper extremity harness for a patient with axillary pressure issues and figured that now was the time to throw this idea out even if we had time to try it basic idea is to use a sort of figure 9 harness only with fingers and no loop--maybe it should be called a figure M as well as a minimal lower harness with sort of backplate made of some plastic sandwiched between a couple pieces of nylon to support the t-bar the backplate is just pieces of This type of lower harness is what Advanced Arm made for me at Walter Reed. They also made me a friction buckle so that my figure 9 can be disconnected from the cable. This is although I have some issues with the lower harness flopping around and my elbow coming out of which I think can be solved with another loose strap over the inside of the forearm. Iany the idea is to use this same type of lower but to have the anchor sewn into an underarmor shirt wicking endorsement I think that it might be possible to anchor the harness to the shoulder with a series of pieces of strapping sewed to the shirt a little before so that the shirt can still stretch perpendicular to the direction of harness of like webbing oa but provide an anchor in the direction of pull duck I am hoping that the friction of these pieces of webbing and the shirt against the skin will be enough to allow the harness to wraps over the top of the shoulder and around side of torso but not under and anything around the axilla. I have posted a drawing here to show what talking about. Another idea is a very large axilla say 3-4 inches with a similar method to try and spread out the forces so that it and effectively turn into a smaller strap. Imagine the laces and holes in a corset that effectively do this. Perhaps an axilla loop could be made out of material such as is used in fancy dress making. Maybe wide loop could be sewed into the shirt. The problem with this is that you lose the possibility of stretch in any direction. Maybe some unidirectional stretch material like that used in the ossur iceross sleeve that could limit stretch ithe direction of harness tension. Just a thought. be interested in hearing what you all
