MySQL issues during setup

When I load openphoto for the first time and try to setup Mysql I get the following error:

Please fix the following errors 
We were unable to initialize your account in MySql. 
Please verify that the host, username and password are correct and have proper permissions to create a database. 
Make sure your email address is not already in use.

How do I solve this issue? Where do I find the logs?

I am able to login the mysql command line app and use the database I setup for openphoto but setup fails when I use the web setup for openphoto

It should output messages to Apache's error log. Can you include what you find there?

Andrew,

We're wrapping up testing a certified build. If you continue to have issues with that then let's revisit then.

That should be available next week. I've tested fresh installs and need to test upgrades next.

I have just installed the new version and I'm have the same issues. What is causing the "Undefined variable" errors?

Stop the setup process after creating the first 4 tables.
