Dear Addiction Treatment Provider

My name is Christopher Russell.  I am a PhD. Psychology student at the
University of Strathclyde, Glasgow, Scotland.  I am currently researching
addiction treatment providers’ beliefs about addiction and their spiritual
beliefs and would be very grateful if you could spare 10 minutes to answer
some questions at the web link below.

You will also be asked some demographic questions and some questions about
your own substance use and abstinence.  Feel free to skip over any questions
you don’t wish to answer.  Any data you provide will remain confidential and
anonymous and is stored securely at all times.
