Thunderbird Version 2.0.0.23 (20090812)
Running on Windows 7 x64 RTM.
I have several e-mail and nntp accounts configured, each is a
'top-level' account, or whatever it's called; ie they all have their own
folder structure in the folders pane.
When I go to compose an e-mail or nntp message, I cannot click the
Address book 'card' icon in the "To:" line. It is greyed out.
I can get to the address book by clicking the large "Address Book" icon
on the icon bar no problem.
Why might that be?

Ah, it's not a clickable icon at all, is it? It's just a stupid
picture of a greyed out address card.
You need to hit the big 'contacts' icon to open the address book in the
Left Hand pane, and dbl-clk them there.

Am I wrong?
Is it meant to be an active icon in the "To:" field, or is it really
just a stupid picture?

Actually, it's not big, it's small ( compared to the icons in the icon
bar ).
And it's not an 'icon' in the usual User Inetrface sense, since it
doesn't do anything ( AFAICS ).
But I'd agree it's stupid.
And we already know what goes here :-)
