Number of Girls Knee Injuries Rising Sharply

As a physical therapist specializing in sports medicine and rehabilitation
for the University of Texas Medical Branch, I work with injured athletes
every day, and Ive found todays new cadre of young female athletes is
particularly prone to a knee condition called patellofemoral pain
syndrome.

Patients with PFPS experience pain in the front of the knee when doing
repetitive activities like running, squatting and climbing stairs. In more
chronic conditions, individuals will complain of pain as a result of
sitting for prolonged periods.

PFPS affects 15 to 33 percent of all active adults. Of all PFPS cases,
between 21 to 45 percent first occur during adolescence.

.

Many reports indicate a sharp increase in cases of PFPS in teenage girls
during the last 20 years.

.

It seems quite possible the dramatic increase in female participation in
sports since the passing of Title IX in 1972 has something to do with
this. A generation ago, the majority of adolescent PFPS cases were among
young men who played rough sports like football.

The Womens Sport Foundation listed a 940 percent increase in sports
participation among high school girls between 1972 and 2008. More than 3
million young women are estimated to be active in sports today. With so
many girls engaging in sports during many years of their young lives, the
percentage of adolescent female athletes who develop PFPS now surpasses
the percentage of teenage male athletes who do.

Theories abound about why girls are more susceptible to this injury than
boys.

.

The Q-angle is one of the most commonly studied and debated knee
conditions. It is the angle created by the intersection of the axis
parallel to the femur and the axis created by the orientation of the
quadriceps, the group of muscles in the front of the thigh. The greater
the angle, the greater the stress placed on the knee during strenuous
activity, increasing the risk of injury. Women have an increased angle
compared to men because they have anatomically wider pelvises.

Studies of pre-pubescent children show there is little variation in leg
mechanics between boys and girls, and also fewer instances of knee pain.
With the onset of puberty, however, an alteration in mechanics occurs but
the change is much more problematic for females.

.

The incidence of PFPS among girls who play sports goes up dramatically
when they go through puberty. This could be because of the anatomical
changes in the pelvis, muscle size or even hormonal influence on the
tissues themselves.
