The post below is my response to a request for the use of the Coaching
Issues Survey in research publications.  Databases tried as noted in my
post below found no occurances of this title in the content they search,
but Google and Google Scholar found a total of four sources that list this
survey instrument.

I have added the links to the documents cited using right click, copy shortcut
to get the three URLs above.

Incidently, this was more productive than a search of quite a few databases
including SportsDiscus that all came up with a null result.

So it may be wise for those wanting more complete search results to Meet the
Googles:

By the way, despite cites from Scholar turning up with dissertations as
sources, Dissertation Abstracts turned up with a null result on this search.
As inaccurate as Google can be as a search tool, this is docmentable, but
another topic, Dissertation Abstracts is searching only the title, any keyword
indexing and the abstract, whereas if the author or the college or some elves
place a complete copy of the dissertation on the internet full text, Google is
quite likely to find this and find the name of the instrument in one or another
line of the full text of the dissertation in those instances that the
dissertation is full text on the web.  As one might say in sports, advantage
Google.

Speaking of Google, there is always the unscholarly Google (Web Search), which
is not part of Meet the Googles, except in the post that covers search tips
that apply to the entire Google family, and this unvarnished Google also seems
able to find something to add to the orchestration of this instumental pot.
